Resources Neuropsychological Associates can be of assistance...how
do I proceed?
Call us at (608) 242-7160. If our front office is open, you will
be asked to provide some basic referral information to assist
us with arranging your first appointment...If you call when our
front office manager is not available, you will automatically
be connected with our voice messaging system. Please leave a message
for a specific clinician, or first-time callers can leave a general
message in box #1.
Some of the basic information you will be asked includes:
- What services are being sought?
- Who is referring you?
- Do you have a specific clinician in mind?
- What is your name, address, contact and insurance information?
- What symptoms, problems, or clinical issues are prompting
the service request?
- What treatment history is available?
- What developmental history is available?
- What previous testing
has been done, by whom, when, & who
has results (for evaluation patients); e.g., to receive
educational accommodations at school, or psychological or
neuropsychological testing?
